Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Alina Nazar

MD

Biography

Dr. Alina Nazar is a bilingual board-certified family physician fluent in Ukrainian, Russian, and conversationally fluent in Polish. She is a member of the American Board of Family Medicine. Dr. Nazar received her medical degree from National Pirogov Medical University in Vinnytsia, Ukraine, and completed a family and emergency medicine residency at M. Pirogov Regional Hospital in Vinnytsia, Ukraine. She also completed a family medicine residency at Ellis Hospital in Schenectady, New York. Dr. Nazar is interested in preventive medicine, screening disease, and women’s health. She likes to consult patients on healthy lifestyle changes such as diet and exercise.

As a child, Dr. Nazar spent a lot of time in hospitals. She received excellent care from physicians and nursing staff which inspired her to become a doctor. Dr. Nazar chose to pursue family medicine because it gives her the ability to provide comprehensive health care for people of all ages, from newborns to seniors. She loves that she gets to care for the whole family and develop long-term relationships. When caring for patients, she believes the most important thing is communication and looking at them as a whole. Dr. Nazar enjoys listening to her patients and taking the time to get to know them and their families. What brings her the most satisfaction professionally is seeing her patients get better, improve their mental and physical health and enjoy their life. During Dr. Nazar’s residency training, she learned from her favorite attending physician to treat patients as your family members, and that is something she still does today. She loves helping her patients live happier and healthier lives. Dr. Nazar hopes to make a difference in her patients and will be available and helpful to them anytime they need.

Dr. Nazar is originally from Ukraine, immigrated to the United States, and moved to the Albany area to further her medical training. She decided to stay in the Capital Region since it’s a great area to grow a family! Dr. Nazar enjoys biking, hiking, riding her bike, playing basketball, and spending time with her daughter and dog.
