Community Care Endocrinology opens third location in Clifton Park

We are excited to now offer three Community Care Endocrinology locations across the Capital Region for endocrinology care (including thyroid and non-thyroid care), diabetes education, medical nutrition, and weight management. Our endocrinology division recently welcomed two new practitioners, Jessica Riggins, FNP-BC, and Grace Hanlon, RD, MS, CDN, CDCES. With an expanded team of specialists, we opened another location in CCP’s multispecialty medical building in Clifton Park.

Community Care Physicians’ endocrinologists provide comprehensive endocrinology care for adult patients with diabetes, metabolic disorders, thyroid disorders, pituitary disorders, adrenal disorders, and other endocrine-related disorders. Additionally, these practices include certified diabetes educators and registered dietitians as part of our team who provide diabetes education, medical nutrition therapy and weight management services, insulin pump education, and more in-person and virtually for our patients. Our endocrinology specialists are well-known in the patient and clinical communities, and we have convenient locations in the Capital Region. Our locations also offer additional on-site services, including medical imaging and lab, for your convenience.

Endocrinology and diabetes education services are provided to patients 18 years and older but will see children five years and older for medical nutrition therapy for disorders such as failure to thrive, food allergies, overweight/obesity, oncology support, specialized diets, and more.

We are excited to expand access to our endocrinology services to our patients in the Clifton Park area. Please contact your primary care provider for a referral to schedule an appointment.
