On February 3rd, CapitalCare Family Practice Niskayuna will be moving to 501 New Karner Road in Colonie where they will provide care under their new name – Community Care Family Practice Colonie.
If you have an appointment scheduled before February 3rd, then please continue going to the Niskayuna office. If you have an appointment scheduled on or after February 3rd, please go to their new office location in Colonie.
We are excited for the move as it offers our patients many advantages. This new office is conveniently located only 5 miles from their current practice in Niskayuna. It is also much more accessible; the suite is all on one floor, the first floor of the building. There is ample, well-lit parking. There are also no stairs for patients to climb, no ramps required, and the building is handicap accessible. This new modern office space will give our team room to expand and grow in the future to better serve our patients.
In addition to the move, we also have a few practitioner changes happening within this practice. Dr. Eugene Haber and Peggy Sheaffer, PA-C retired at the end of the year, and Allison Godfrey, FNP-C will be leaving the practice on February 6, 2020. We thank them all for the great care they have provided our patients over the years. As we bid them farewell, we are excited to welcome Alexis Hargrave, PA-C to their team! She has already started seeing patients at this office.
If you have an upcoming appointment with one of the practitioners who are leaving, the office will call you to help you reschedule with another practitioner.
Family Practice Niskayuna will also be temporarily suspending walk-in hours so they may continue seeing their scheduled patients while they prepare for their move to Colonie. Rest assured, once they are settled in their new home, they plan to once again offer walk-in times! So, if you need to be seen, we ask that you please call the office for an appointment. Their phone number (518) 393-0391 and fax number (518) 372-3281 will not be changing in the move. Please know that with all of these changes, Family Practice Niskayuna is experiencing a higher volume of calls than usual. If you do call our office for any reason, please be patient. Our team is working diligently to address your needs as soon as possible.
