Community Care Physicians offers patients a diabetes education program as part of our weight management program at Community Care Physicians. Our diabetes education program will help you gain the knowledge, skills and confidence to thrive with diabetes. The program’s certified educators are ready to work with you to find practical solutions that fit your personal needs. Our program offers education for Type 1, Type 2 and Gestational Diabetes, insulin starts, pen teaches, diabetes management-related learning, and self-management. This program is primarily for adults, but some teens are seen as needed. Diabetes education services are typically covered by insurance as part of our patients’ medical care.
