Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Flu season is here, can you believe it? Give it your best SHOT this flu season!

We want to make it easy for you to get your flu vaccine, so Clifton Park Family Medicine is running flu clinics throughout the next month for our patients 6 months and older. If you’d like to attend one of our flu clinics, please schedule an appointment and we can get you in and out.

Monday October 17^th, 6pm – 7pm
Wednesday October 19^th, 6pm-7pm
Saturday October 22^nd, 9am – 12pm
Tuesday October 25^th, 6pm – 7pm
Thursday October 27^th, 5pm – 6pm
Saturday October 29^th, 9am – 12pm
Every morning @ early walk-in from 7am – 9am between the 24^th – 28^th

If you already have an appointment scheduled with Clifton Park Family Medicine, you can also ask for your flu vaccine at that time. Otherwise, we encourage you to take advantage of our upcoming flu clinics! Call us today at (518)-383-2366.
