Biography

Tiffany Schermerhorn, AGACNP-BC, is a board-certified Adult-Gerontology Acute Care Nurse Practitioner specializing in weight management and disease prevention. She brings nearly 20 years of experience in critical care nursing to her role at Weight Management & Metabolic Care.

Tiffany earned her Bachelor of Arts in psychology from Siena College in Loudonville, NY, before pursuing her Bachelor of Science in nursing at Russell Sage College in Troy, NY. She continued her education at Russell Sage College, earning a Master of Science in nursing. She is certified by the American Nurses Credentialing Center (ANCC).

Originally from Clifton Park, Tiffany has deep roots in the Capital District, where she remains close to her family. Her clinical background and passion for patient-centered care have led her to focus on disease prevention and weight management. She believes in a collaborative approach between patients and providers, emphasizing open communication, education, and compassionate care to help individuals achieve their health goals.

In addition to her clinical practice, Tiffany is an Assistant Professor of Nursing at Russell Sage College, where she helps shape the next generation of nurses. She is a member of the American Association of Critical Care Nurses (AACN) and the American Nurses Association.

Outside of work, Tiffany enjoys traveling with her husband and two sons and reading. Her greatest professional satisfaction comes from knowing she is making a meaningful difference in the lives of her patients.
