EMR Upgrade throughout the weekend: CCP will be completing a scheduled upgrade to our electronic medical record (EMR) system to help enhance and support your care experience. Our practices remain accessible, on-call providers continue to be available, and Latham Urgent Care will remain open for urgent medical needs. Most of this work is happening behind the scenes. If you receive care this weekend, you likely won’t notice any difference, other than occasionally confirming information to ensure we have the most accurate details for your care. We appreciate your patience.

 

If you visit Urgent Care this weekend, it would be helpful to bring a current list of your medications. You should also expect paper prescriptions rather than electronic prescriptions during portions of the upgrade. Any other office-specific changes or instructions will be communicated directly by your practice, if needed. Please check your practice’s website for the latest updates.

Lisa Vergine

Biography

Nurse practitioner Lisa Glantzis, originally from Schenectady, earned her Bachelor of Science in nursing from SUNY Polytechnic Institute in Utica, NY. She proceeded to receive her Master of Science, family nurse practitioner also from SUNY Polytechnic Institute. Lisa is a member of the American Association of Nurse Practitioners and the Association of Rehabilitation Nurses.

For over 35 years, Lisa has worked with geriatric, pediatric, and rehabilitation populations for most of her career as a registered nurse. With all her years of experience in the field, Lisa knew she had more to offer, so she decided to go back to school and become an advanced practitioner. What Lisa loves most about working in the rehabilitation and internal medicine setting is helping her patients with more than a diagnosis and treatment. She gets to develop a relationship with her patients and their families and has the opportunity to educate them on their health issues. Lisa is interested in pediatrics, traumatic brain injuries, and stroke.

When it comes to caring for patients, Lisa believes treating each patient holistically, reviewing their medical history, and having trust and communication is most important. At Sunnyview Hospital and Rehabilitation Center, Lisa’s job is to treat patients with rehabilitative issues, as well as internal medicine concerns. Her duties include evaluating patient history, performing physical exams, writing physician orders, daily treatments, etc.

Outside of work, Lisa enjoys cooking, gardening, and spending time with family and friends.
