Some of our patients have received letters in the mail from Change Healthcare regarding a data breach.
Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Dr. Kristen Martin is board certified in family medicine. She received her medical degree from MCP-Hahnemann School of Medicine, Pennsylvania, and completed a family medicine residency at St. Clare’s Hospital in New York. Dr. Martin enjoys serving the community she grew up in and caring for multiple generations within the same family.
