Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Joelle Malinowski is a highly qualified professional in the field of nutrition. She is a registered dietitian, a certified dietitian-nutritionist, and a certified diabetes care and education specialist. Her extensive education includes a Bachelor of Science in dietetics from the State University of New York, College at Oneonta in Oneonta, NY, and a comprehensive dietetic internship at North-Shore LIJ Hospital in New York, NY. Joelle is a member of the Academy of Nutrition and Dietetics and the Association of Diabetes Care and Education Specialists. Her expertise lies in medication nutrition therapy for weight loss, diabetes, hypertension, elevated cholesterol, disordered eating, IBS, IBD, kidney disease, and celiac disease, among others.

Originally, Joelle was on track to study broadcasting but never was in love with it. She recalls gaining the infamous “freshman 15” and working on lifestyle modifications to lose weight. She quickly fell in love with the field of nutrition and education and switched career paths. What Joelle loves most about her job is helping patients understand their current medical conditions. She likes to develop fun and exciting teaching methods to help them learn complex medical information. Joelle finds that the more patients understand their medical condition and treatment options, the more success they have in increasing the general health and quality of their lives. Along with helping patients with their health, she loves getting to know them and learning about their lives, jobs, families, and hobbies.

Joelle firmly believes that patient-centered care is the cornerstone of her practice. She understands the importance of setting realistic goals that align with each patient’s unique health journey. By working with patients to overcome barriers and tapping into their strengths, she creates reachable and sustainable goals. Joelle aims to foster a comfortable, fun, motivating, and judgment-free environment, where every patient feels valued and understood.

Joelle was born and raised on Long Island. Outside of work, she is involved in community theater in the Capital Region and has performed in many musicals and plays since moving here in 2018. She has been doing community theater since she was eight years old! Joelle’s love for the arts and performance has taught her the importance of balance and self-expression, which she incorporates into her approach to nutrition and weight management. She believes in the importance of enjoying life and finding joy in simple pleasures, which she encourages her patients to do. She enjoys staying active at the gym and spending time with her husband, daughter, and dogs.
