Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Eleanor Fiacco

Biography

Eleanor Fiacco is a board-certified family nurse practitioner specializing in family medicine. She received her Master of Science in nursing from Russell Sage College in Troy, NY. Eleanor began her nursing studies at SUNY Adirondack in Queensbury, NY, earning her Associate of Science in nursing. Following this, she completed her Bachelor of Science in nursing from SUNY Delhi in Delhi, NY. Eleanor also received a Bachelor of Science in biology from Sage College of Albany in Albany, NY. Eleanor loves being a family practitioner as she has the opportunity to help patients across the lifespan, but she has a particular interest in caring for the pediatric population.

Eleanor always wanted a career in medicine as both of her grandmothers were nurses. Eleanor enjoys working with patients and their families, but her absolute favorite thing is making her pediatric patients who are scared or nervous to be at the doctor’s office smile and laugh by the end of the visit. She believes kids understand more than we think they do, so helping them understand without being scared is rewarding to her. Eleanor enjoys educating patients about their medical needs and building plans to improve their quality of life.

Eleanor believes building trust and keeping an open line of communication is most important when caring for her patients. As a happy and energetic FNP, she cares about helping others and making them feel better.

Eleanor was born and raised in Delmar, NY, and loves living in upstate NY as it is close to the mountains and lakes and only a short trip to the ocean and major cities! She has previously volunteered and worked as a staff RN at Double H Ranch, a camp for children with life-threatening illnesses, in Lake Luzerne, NY, since 2016. Outside of medicine, Eleanor enjoys reading and spending time with the small book club she and her sister started five years ago. She also enjoys hiking with her family and puppy and spending time on the lake. Eleanor’s Nana bought her first sewing machine when she was ten years old, so some say she is an amateur seamstress – she has made a few skirts, dresses, baby blankets, changing pads, and even a stuffed animal!
