Creig Hayes, FNP-C, is a Family Nurse Practitioner providing adult primary care at Community Care Physicians’ Delmar practice. He is committed to delivering personalized, compassionate care that focuses on the whole person, helping patients achieve their health goals through prevention, education, and evidence-based treatment.
Creig earned his undergraduate nursing degree from Excelsior College in Albany, New York, and completed his graduate education at SUNY Polytechnic Institute in Utica, New York, where he became a Family Nurse Practitioner. He cares for adult patients and is passionate about building long-term relationships that support lifelong health and wellness.
Originally from Berlin, New York, Creig chose to make the Delmar area his home while building a life and raising a family. He values the opportunity to care for community members and to develop meaningful relationships with the patients and families he serves.
Creig’s path to healthcare began unexpectedly. Initially pursuing a degree in physical education, he soon realized his passion was helping others and making a meaningful difference in people’s lives. That realization led him to nursing, where his experiences caring for patients and forming strong connections inspired him to advance his education and become a nurse practitioner.
His clinical background is rooted in cardiac care, and he has a strong interest in disease prevention, nutrition, exercise, and overall wellness. He believes that lasting health is achieved through a balanced approach that includes physical activity, healthy nutrition, mental well-being, and preventive care. While medical treatments and interventions are important, he is passionate about helping patients make sustainable lifestyle changes that improve their long-term health outcomes.
Creig’s approach to care centers on trust, communication, and collaboration. He believes the most important aspect of healthcare is taking the time to understand each patient’s unique concerns, educating them about their health, and ensuring they feel heard, respected, and involved in decisions about their care. During his training, Creig particularly enjoyed his sports medicine and spine care rotation, where he gained valuable insight into alternative approaches to pain management, rehabilitation, and preventive care. He also credits a clinical rotation with a nurse practitioner at Community Care Physicians for reinforcing his commitment to patient-centered medicine and for demonstrating the collaborative, compassionate practice environment he values.
Creig finds the greatest professional satisfaction in knowing he has made a meaningful difference in a patient’s life, whether by helping them improve their health, manage a condition more effectively, or simply ensuring they feel supported throughout their healthcare journey.
Outside of the office, Creig enjoys staying active and maintaining a healthy lifestyle. His experience across a range of fitness activities has contributed to his balanced perspective on health and well-being.
