Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Biography

Casey Feldman is a board-certified family nurse practitioner with several years of nursing and direct patient care experience. She received her Master of Science in nursing, family nurse practitioner, from Russell Sage College in Albany, NY. Casey began her nursing studies at Hudson Valley Community College in Troy, NY, earning her Associate of Applied Science in nursing. She also received a bachelor’s degree in marketing from Siena College in Loudonville, NY. Casey has a particular interest in health promotion, including nutrition and exercise education, helping children manage chronic health conditions, and adolescent health. Casey has spent seven years as a pediatric oncology nurse and had the opportunity to see and appreciate the strength and resiliency of children. The experience provided her with invaluable exposure to the management of chronic conditions and further highlighted the importance of promoting family-centered care.

Casey loves the patient education aspect of her job, answering questions and teaching patients and families how to manage their health conditions properly. She believes adequate communication is key; listening to the patient and parents’ concerns and having a thorough understanding of the patient’s history helps facilitate better care outcomes.

Patient advocacy is of utmost importance to Casey. She will always strive to ensure that the patient and their individual needs are being met. She says, “My goal as a practitioner is to provide quality, patient-centered care while building long-standing, trusting relationships with my patients.”

In her spare time, Casey enjoys traveling, reading, cooking, and running.
