Dear Patients,

I am writing to inform you of my retirement from the practice of medicine in Niskayuna effective January 2022. Caring for patients for over 30 years has been a very rewarding part of my life. It’s been a pleasure and honor to serve you over the years with CapitalCare and now as part of Community Care Physicians

It is time for my family and me to move on to the next chapter of our life. We will be moving permanently to our house in the northern part of the Adirondacks in few months. We also look forward to spending more time with family both in the U.S. and abroad in Belgium.

We are excited to welcome a new physician to our practice, Nicholas Fusella, DO. Dr. Fusella, who is from the area, will be joining our practice in mid-January and is looking forward to continuing your ongoing care. You will not need to transfer your medical records because they are available in our shared medical record. Dr. Fusella has access to your pertinent medical information for continuity of care.

I want to thank my staff, my previous colleague Dr. Smitas and our Adult Nurse Practitioner Christina McGaffin for their years of commitment to the practice and patients.

Please be advised that should you decide not to receive care from the physicians at Community Care Physicians; your medical records will be made available to the physician you designate. Since these records are confidential, Community Care Physicians requires your written authorization to make them available to another physician. Any questions regarding medical records can be directed to our office at (518) 280-8470.

I very much enjoyed being your physician and wish you the very best of health in the future.
