Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

When you are admitted to Albany Medical Center, you may be cared for by one or more Community Care Physicians (CCP) hospitalists during your stay. Our goal is to ensure that Community Care Physicians patients receive the best care possible in the hospital, mirroring the standard of care you receive in our Community Care offices.

CCP hospitalists at Albany Medical Center are providers specializing in inpatient care who will lead the medical team and coordinate care while you are in the hospital. Our comprehensive team includes physicians, advanced practitioners, care managers, and patient navigators who work closely with your primary care practice to ensure great care in the hospital and a smooth transition following your hospital discharge.

Your CCP hospitalist will communicate with your CCP primary care provider in a timely manner, so all providers involved in your care have the information they need. Discharge planning starts at admission. Our team will communicate regularly with your primary care office to ensure a seamless transition.

Who is this program for?

CCP’s hospitalists care for CCP adult patients of our primary care offices who are admitted to Albany Medical Center.

Remember, if you have a medical problem that is not an emergency or life threatening, please call your primary care office first before visiting an emergency room or urgent care. Your primary care provider may be able to resolve your problem or to direct you to the proper level of care.

If you and your doctor decide you need to go to the emergency room, we encourage you to choose Albany Medical Center, when possible. When you arrive at Albany Medical Center, you should tell them you are a patient of Community Care Physicians.

Who to call and when:
  • If you were seen by a CCP hospitalist at Albany Med and have urgent questions between your hospital discharge and when you are scheduled to see your primary care provider, call our hospitalist team at (518) 971-9050.
  • If you are scheduled to be seen at the Transition of Care Clinic in Albany following your hospital stay, call us at (518) 881-0780.
  • If you have been seen by your primary care provider for follow-up care, please contact their office with any questions.
