You can now easily request your medical records online! Community Care Physicians (CCP) has a new online platform for patients, authorized patient representatives, and authorized attorneys to effortlessly request medical records, eliminating the need to visit us in-person or spend time on the phone requesting this information. We have partnered with Verisma to fulfill online requests for records and save you time. All requests are fulfilled within five (5) to ten (10) business days of receipt of the request, and you can choose to receive your medical record securely via email, on paper in the mail, or on CD/DVD. You can also very easily check the status of your request using a tracking number provided through our new online system.

All patient health care information at Community Care Physicians is confidential and protected by State and Federal laws and HIPAA regulations. Medical records can only be released with proper authorization from the patient or the patient’s legally authorized representative (unless otherwise authorized by law). 

The majority of CCP practices offer this convenient online option, with a few exceptions. The following practices currently do not accept online requests for medical records:

• Community Care Urology – Latham
• ImageCare (all locations)
• Image-Guided Radiation Therapy
• The Audiology Group (all locations)
