Cynthia Stone, DBH, LMHC, NCC

Director of Integrated Behavioral Health of Community Care Physicians

Dr. Cynthia Stone is the Director of our Integrated Behavioral Health program. This program provides mental health, substance use, and behavioral health care for the patients of Community Care Physicians by embedding behavioral health providers within our practices and linking patients to specialty mental health care they may need within our community. Dr. Stone earned her Doctor of Behavioral Health at Arizona State University, College of Health Sciences. Dr. Stone is a member of the American Psychiatric Association, Collaborative Family Healthcare Association, and the American Counseling Association.

Dr. Stone has always had a strong interest in psychology, human behavior, wellness, and helping others live their best life possible by overcoming and managing difficult issues and life stressors. Coinciding with her interests, she acknowledges the mind-body-spirit connection and the effect one system has on the other. Experienced in mental health counseling and private practice, along with fitness and wellness training, she now finds multiple benefits in integrated primary care using the team approach to treating her patients. Dr. Stone is additionally excited to be involved in the proliferation of both the clinical and organizational components of integrated behavioral health care as she is active in research, presenting, and publishing.

 

Jenna Gazzillo, LMHC

Jenna is a Licensed Mental Health Counselor and works as a Behavioral Health Consultant (BHC) on our Integrated Behavioral Health team. BHCs at Community Care Physicians are licensed mental health providers and social workers. Many are embedded within our practices working side by side with your doctor and health care team in delivering you the best “whole person” healthcare possible.

Jenna has experience working with children and their families. She is a member of the New York Mental Health Counselors Association (NYMHCA). Throughout her education and training, she found that individualized treatment and collaborative care are critical in the behavioral health field.

Jenna decided she wanted to be in the mental health field during her undergraduate years. However, it wasn’t until her internship that she solidified her decision after learning more about behavioral health and its importance. When Jenna first began as an intern, providing behavioral health services to children and their families, she realized how much she enjoys working in the field and how fulfilling it was. What she found most interesting during her training is learning about child behavioral health and how counseling differs between children and adults. Jenna loves being able to do something she is so passionate about while also helping others improve their overall quality of life.

Jenna believes listening and paying attention to her patient’s needs is the most important thing she can do. She finds working with children most rewarding because she thinks they are such a vulnerable population. She likes helping them identify the necessary tools and skills that can help them for a lifetime.

In her spare time, Jenna enjoys outdoor activities with her dog, has a strong love for music and singing, and spending time with family and friends. She is also a member of the Schenectady Veterans Club.

 

Kristine Campagna, DO

Dr. Kristine Campagna is a board-certified family physician at Latham Medical Group of Community Care Physicians. She holds a Certificate of Added Qualification in sports medicine. She received her Doctor of Osteopathic Medicine from New York College of Osteopathic Medicine (NYCOM) in Old Westbury, New York, and completed her osteopathic rotating internship and residency training in family medicine at Albany Medical Center in Albany, New York. Following residency training, Dr. Campagna completed a primary care sports medicine fellowship at Ohio State University in Columbus, Ohio. She also received an MBA in healthcare management from Clarkson University. Dr. Campagna began her studies at Columbia University in New York City, New York, earning her Bachelor of Arts. She has a particular interest in sports medicine. Dr. Campagna enjoys doing procedures like corticosteroid injections that help lessen patients’ symptoms.

Dr. Campagna decided to pursue medicine after a sports-related injury in high school. She went to physical therapy for several months became interested in treating sports-related injuries. Dr. Campagna’s favorite part of being a family physician is the relationships with her patients. She loves helping them achieve a goal, whether it’s weight loss, better glycemic control, or improved mental health.

Dr. Campagna grew up in Garden City, New York. As a physician who enjoys the practice of medical principles related to sports science, it’s not a surprise that she also enjoys sports outside of the office! Dr. Campagna likes weight training and spending time with her husband and two sons.

 

Holly Cleney, MD

Dr. Holly Cleney is a board-certified family physician at Latham Medical Group of Community Care Physicians. She received her medical degree from SUNY Health Science Center in Syracuse, New York, and completed a family medicine residency at Albany Medical Center in Albany, New York. Dr. Cleney began her studies at Binghamton University in Binghamton, New York, earning her bachelor’s degree.

Dr. Cleney has a general interest in helping people understand how so many of our physical symptoms come back to stress and how we can worry so much less about our health if we can understand that. Moreover, helping people with their struggles with losing weight is challenging but rewarding when success happens, and people adopt more healthy lifestyles and see its benefit.

Dr. Cleney had the opportunity to speak throughout the country about Latham Medical Group’s work with behavioral health integration and transforming medical care to be higher quality and more patient-centered. She loves working in family medicine because she gets to know people of all ages and families of all types and becomes part of improving or maintaining their health and happiness. She values having the chance to be in that process and share their joys and grieve with them in all stages of life. Dr. Cleney believes being trusted to be in that position is such an honor.

Dr. Cleney loves that every day is a new day and never knowing what will be on the schedule – from a brand new baby to a procedure, to helping a patient out of a depression or severe anxiety attack, to a cancer to diagnose or to simply a well child to hear their accomplishments. She also enjoys working with patients over time to learn how best to manage their chronic diseases like Diabetes.

Dr. Cleney is on the board of Building on Love, a non-profit organization offering support to local families facing medical emergencies, and serves on the Board of Directors of CDPHP. In her spare time, she enjoys fitness classes involving weight lifting, basketball, and golf. Also, you will likely find her cheering for the Buffalo Bills when she’s not cheering her three children on in their volleyball, basketball, baseball, and soccer games.
