Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Leah LaFrance

Biography

Leah LaFrance is a Registered Dietitian and a member of the Academy of Nutrition and Dietetics. She received her Bachelor of Science in nutrition from the State University of New York at Plattsburgh in Plattsburgh, NY. She completed her distance dietetic internship and graduate certificate program through Iowa State University. Leah treats patients 18 years of age and older. She has been in weight management since 2022 and enjoys working with this population. Leah also enjoys teaching others about prenatal nutrition and breastfeeding. She has a passion for this after working at the WIC program and going through her own pregnancy and breastfeeding journey.

After graduating with her undergraduate degree, she moved to Florida to gain experience in the nutrition field. While in Florida, Leah worked for the WIC program and LTC facilities and completed her distance dietetic internship. Once she became a certified dietitian and had more experience, she moved closer to home to be near family and work in an area with ample opportunities.

Leah decided to pursue nutrition during her senior year of high school after shadowing a Registered Dietitian in the Allied Health Program through BOCES. This experience allowed her to shadow various medical fields before attending college. Leah liked that there were multiple specialties that she could pursue within the nutrition field. What she loves most about her job is helping patients improve their quality of life. She believes getting to know each patient as an individual is important. In the weight management setting, Leah loves that patients are looking to improve their quality of life for the better. She creates individualized plans for each person to achieve their weight loss goals. Leah enjoys seeing patients progress and what weight loss achieves, both physically and mentally.

Leah is originally from Massena, NY. Outside of nutrition, Leah enjoys going to the gym and Zumba, going for walks with her family, and spending time by the water.
