Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Asma Arif

Biography

Dr. Asma Arif is a double board-certified physician in internal medicine and gastroenterology. She is a member of the American Society of Gastrointestinal Endoscopy and the American College of Gastroenterology. Dr. Arif received her Bachelor of Medicine, Bachelor of Surgery from Fatima Jinnah Medical College in Lahore, Pakistan. She completed an internal medicine and pediatric residency at SUNY Upstate Medical University in Syracuse, New York. She furthered her training as a gastroenterology Fellow also at SUNY Upstate Medical University. Dr. Arif served on the faculty of SUNY Upstate as an Assistant Professor of Medicine before starting her private practice in the Capital Region over the last 16 years.
