Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

WTEN - Community Care Physicians (CCP) and Capital District Physicians’ Health Plan, Inc (CDPHP) have announced a merger between the companies. The merger is targeted to close by December 31 after final approvals by certain state agencies.
