Provider Application

  • Diverse Practice Settings

CCP has a wide range of practice environments in primary care and specialty care, from small, close-knit solo provider offices to large, multi-provider groups and residency program sites. In any setting, you will practice to the full extent of your license.

  • Supportive Environment

You have access to over 30 CCP specialties for coordinated patient care. Our structure provides strong peer support for collaboration, mentorship, and continued professional growth. You also have access to centralized administrative, clinical, and technological support (including HR, operations, IT, legal, billing/coding, finance, etc.)—so you can focus on your patients.

  • Local, Provider-Led Organization

CCP is a provider-led, local, independent private practice model. Your voice is valued and heard. You have a high degree of autonomy in how you practice and many opportunities for growth and leadership. Our organizational structure enables CCP to remain agile and innovative to improve the patient and provider experience.

  • Strong Work-Life Balance

Work-life balance isn’t just a talking point. As a non-hospital setting, our providers enjoy desirable schedules, such as no hospital rounding, except in select specialty practices that care for hospitalized patients. As a large organization, we offer providers both set/predictable full-time opportunities as well as flexible/part-time shifts if that better suits your needs; many CCP practices offer condensed work weeks (e.g., 4-day schedules) or even float or per-diem schedules.

CCP has many opportunities across primary care and specialty practice settings.

Let us walk you through them. Email us: [email protected]. If you're interested in multiple opportunities, you can also apply using our generic provider application below.
