ALBANY — Community Care Physicians, Capital District Physicians Health Plan (CDPHP) and the Albany Med Health System are teaming to create what officials from all three organizations describe as a collaboration that will improve efficiencies between two of the largest medical practices in the region. “The intent is to remove barriers to access and communication between patients, providers and Albany Med,” said Dr. Ronald Musto, CEO of Community Care Physicians.
