The Pediatricians at Clifton Park Pediatrics are dedicated to providing our patients with comprehensive pediatric care, from birth through to college. We are conveniently located on Route 9 in Clifton Park. Our practice's team of professionals is committed to assisting you and your children with any healthcare concern you may have.

Chances are you'll visit your child's pediatrician most often during the first year of his or her life for routine visits. However, regular visits to the doctor are necessary at every age. We encourage you to speak with your doctor at Clifton Park Pediatrics about what visits are needed at what time in order to maintain optimal health for your child.

Our staff is specially trained to get you the answers you need to assist with your family's healthcare needs. Our nurses are committed to educating patients about their medical conditions and working with them to reduce their health risks. Our office focuses on prevention and disease management, in addition to treating acute issues that arise.

During non office hours an on-call pediatrician is always available. We are also affiliated with local Community Care Physicians' Urgent Care Centers if you need to be seen at a time when our office is closed.

You can schedule appointments by phone or in person and, if you are a patient of our practice, you can also utilize our secure patient portal, mycareDOT™ powered by FollowMyHealth™, to request or change an appointment. mycareDOT™ is a secure, HIPAA compliant, patient portal that allows you to connect with us remotely (from anywhere), using your computer and internet. Our patient portal is connected directly to our electronic health record and helps you keep track of your medical information. You can request appointments and prescription renewals, message your physician, receive appointment reminders, and even receive some test and lab results.

NICE Weight Transitions Program

NICE Weight Transitions Program

Community Care Physicians has the area’s first weight management program designed specifically for adolescents ages 11 to 18. The NICE Weight Transitions Program focuses on weight management and lifestyle modification by providing your child with access to a health coach, personal trainer, nutritionist, and behavioral counselor to achieve better overall health.

This program is offered free of charge to our patients at Clifton Park Pediatrics. If you are interested in NICE Weight Transitions Program, your child's pediatrician will medically clear your child and will receive regular progress reports as to his/her progress throughout the program. Speak with your pediatrician at our office to discuss if this is a good option for your child.
