Community Care Physicians (CCP) did not have a security incident. Our systems were not compromised.
Change Healthcare, which was previously a payment processing vendor for our billing system, Veradigm, did have a security incident. This past relationship is why CCP was named in letters our patients received.
If you have questions, patients can call Change Healthcare at 1-866-262-5342, Monday through Friday, 8 am to 8 pm, CT. Change Healthcare is offering patients the option to sign up for free credit protection. Please follow the instructions in your letter.

Stephanie Young

PharmD

Biography

Stephanie has been in the pharmacy profession for more than 20 years. In 2006, Stephanie graduated with her Doctor of Pharmacy from Albany College of Pharmacy and Health Sciences in Albany, NY. She completed her PGY-1 residency at the Stratton VA Medical Center in Albany, NY. Stephanie primarily does Medicare wellness visits with CCP’s 65+ population. At these appointments, Stephanie works with the patient to discuss goals and plans to maintain a healthy lifestyle during the later years of life and tries to identify ways for patients to stay involved in their own health care. Comprehensive medication reviews are also conducted to optimize medication safety and adherence. Being able to support CCP’s patients, encourage them, and often feel inspired by them is exactly what she wants to be doing in her career. With a background in pharmacy practice, she can use her skill set and training to help other medical providers keep our patients healthy.

Stephanie knew she wanted to work in healthcare early on, having watched her mother become a nurse. Throughout high school, she was a teen volunteer at Crouse Hospital in Syracuse, NY. Stephanie also spent her senior year shadowing all types of medical providers as a New Vision Allied Health Program student. During that time, she learned that she was specifically interested in the profession of pharmacy. Stephanie focuses on building trusting relationships with her patients. She wants her patients to know that she is their resource, their accountability partner, their supporter, and their advocate. Stephanie values her connections with her patients and colleagues and strives to impact patients’ lives positively.

Stephanie is originally from the Central New York area and settled in the Capital District after college and residency. Stephanie enjoys sports, music, trying new foods, exercising, and spending time with her family and two dogs. Whether on the golf course, basketball court, tennis court, skiing down a mountain, or enjoying a show in a concert hall, she always keeps busy with her three children and year-round activities.
