You no longer need to call our office to make an appointment. Community Care Family Medicine offers an online patient portal, called mycareDOT™, powered by FollowMyHealth™. You can request appointments through this secure, HIPAA compliant system anywhere, anytime from the internet.

Email should never be used to communicate emergency information. If you are experiencing a life-threatening emergency, call 911 immediately.
CCFM does offer a secure patient portal, mycareDOT™, powered by FollowMyHealth™ to connect with our office for general messages, appointment requests, prescription renewals and some lab and test results. Contact us for more information or if you're interested in enrolling.
Please note that email is not a secure medium. Any communication via email should never be considered secure. Sending patient information via email, including clinical or personal information, is not recommended.

Canceling an Appointment
If you must cancel an appointment, we ask that you do so at least 24 hours prior to the time of the appointment. We are happy to reschedule your appointment for a better time.

Please call our office first before coming in if you are ill to avoid the potential of unnecessarily exposing other patients and our staff to the novel coronavirus (COVID-19). While we don’t currently offer walk-in hours, our practitioners are easily accessible to care for you in multiple ways, including over the phone, using a video visit (telemedicine), or in the office while following strict infection control procedures to keep you safe. Whatever your need, you should call our office and our team will determine the best way to care for you.
