Routine appointments can be scheduled on the phone by calling our office, in person following your prior visit, or through our patient portal. We realize you or your child may be sick unexpectedly, so we offer same day appointments and extended hours for sick appointments until 6pm Monday through Friday, and Saturday 9am to 11am.

To make your office visit as convenient as possible, we provide the office forms we need completed for your appointment online. If you have a question as to which forms are needed, please feel free to call our office during business hours and we can help you locate the correct form.

Please call our office first before coming in if you are ill to avoid the potential of unnecessarily exposing other patients and our staff to COVID-19. While we don’t currently offer walk-in hours, our practitioners are easily accessible to care for you in multiple ways, including over the phone, using a video visit (telemedicine), or in the office, while following strict infection control procedures to keep you safe. Whatever your need, you should call our office and our team will determine the best way to care for you.

Preparing for Your Visit

A little preparation can help us serve you better. Please bring with you to your appointment:

  • Your current medical insurance identification card
  • A list of current medications
  • Any medical records from your prior physician if you are a new patient to Community Care Physicians, P.C.
  • If you are a new pediatric patient, please bring your immunization records with you to your appointment if you haven't already mailed them to our office.

Prescription Information

Please bring any pill bottles and list of your current medications with doses to your appointment. This information helps us determine which medications need to be refilled. Most prescription refills can be completed in 48 hours.


Your family medicine provider may recommend that you be evaluated and treated by sub-specialty physicians if your medical condition warrants.

Community Care Physicians includes a number of excellent sub-specialty physicians, all of which share electronic health records and imaging studies allowing your records to be immediately accessed by the providers administering your care. We are happy to refer you to any of these Community Care specialists when needed or to any appropriate sub-specialist whom you prefer.

Our office will provide notifications to, or request authorization from your health insurer if these are required.

We will provide the consultant with your relevant medical history so that the consultant is informed.

We offer referral processing in 24-48 hours.

Hospital Coverage

Your hospital care will be coordinated with an experienced team of specialists in inpatient care who work in close contact with our clinicians and who have access to the important parts of your medical records.

For you, this means personal and efficient care throughout and beyond your hospital stay.
