All Community Care Physicians practices previously located in the Capital Region Health Park have moved two miles away to Wellness Way, a 263,000 square foot cutting edge health park located at 6 Wellness Way, Latham, NY. Conveniently located off Route 9 in Latham, Wellness Way offers a seamless experience between patients, providers, pharmacy, and health plan.
