Niskayuna Family Medicine Welcomes Elizabeth Horn, FNP

Elizabeth (Libby) Horn is a board certified Family Nurse Practitioner at Niskayuna Family Medicine. She received her Bachelor of Science degree in Nursing from Columbia University and her Master of Science degree in Nursing from SUNY Binghamton's Family Nurse Practitioner Program. Libby has experience as an advanced practice provider, caring for patients in primary care office settings, businesses, and health management programs throughout the Capital Region. She was part of the Community Care Physicians family previously, and we are glad to have her back with us! As a Family Nurse Practitioner, Libby sees patients of any age. She has done women's health, pediatrics, adult care, and geriatrics throughout her career, and will continue to do so at Niskayuna Family Medicine. When she is not working, Libby sings competitively in two different Sweet Adeline's choruses.  She once sang against herself in a competition, besting herself by only one point!
