Our pediatricians and nurse practitioner are dedicated to providing the quality healthcare services for your children. As board certified, fully licensed practitioners, we are trained to diagnose and treat a range of acute and chronic medical conditions in children of all ages. As parents ourselves, we understand the concerns you may have surrounding the health and well being of your child. From extra guidance in newborn care and school physicals to maintaining immunizations and well visits, count on us to help you manage your child's health, from infancy through adolescence.

As part of a larger, comprehensive center of care at the Capital Region Health Park in Latham, our practice offers numerous advantages to a traditional pediatric practice. In addition to extended hours of operation, the Capital Region Health Park provides a unique infrastructure of services, capable of supporting most of your child's healthcare needs in a central, quality controlled environment. From laboratory and pharmacy to x-rays, medical specialists, and Urgent Care, the support you need is here for you. As an extension of your pediatrician's own practice, you can expect our colleagues within our Latham location to respond with the same prompt, convenient, and personalized attention as your own pediatrician.

Our office also offers the convenience of extended hours during the week and on weekends. When needs arise and our office is not open, Community Care Physicians, PC has an Urgent Care Center located across the hall from our office at the Capital Region Health Park open during evenings and weekends. Since the Urgent Care Center and our office are both part of Community Care Physicians, your pediatrician at Community Care Pediatrics-Latham will receive your child's notes regarding that visit through our Electronic Health Record if you need to utilize our Urgent Care Center's services.

Thomas Edward Duff Jr., MD

Dr. Duff received his BA from the University of Vermont and his medical degree from the University of Vermont College of Medicine. Dr. Duff's Internship took place at Mott's Children's Hospital of the University of Michigan in Ann Arbor. He completed his fellowship in Ambulatory Pediatrics at the University of Michigan Affiliated Hospital, Wayne County General Hospital, Eloise, Michigan and received his New York State Medical License. In 2000, he went to work at Community Care Physicians, as a Pediatrician for Latham Pediatrics. Dr. Duff is Board Certified in Pediatrics and serves as a Diplomat for both the National Board of Medical Examiners and the American Academy of Pediatrics. Since 2000, he has served as a member on the Board of Directors at Community Care Physicians. Dr. Duff lives in Niskayuna, New York. He has been a long-standing member of the Capital District Pediatric Society and past president. Dr. Duff's interests include skiing, travel, sports, and outdoor activities.

Amanda J. Dvorscak, DO

Amanda Dvorscak, DO is board certified in pediatric medicine. A graduate of the College of the Holy Cross, she went on to attend the University of New England College of Osteopathic Medicine. Dr. Dvorscak later completed her internship and residency at Albany Medical Center. She is an attending physician at Albany Medical Center, St. Peter's Hospital, and Bellevue Women's Care Center. She grew up in the Capital Region and is actively involved in the community working with high school students promoting the healthcare profession. Her profession interests include child advocacy, adolescent care, and obesity prevention. Dr. Dvorscak is a member of the American Academy of Pediatrics and the Capital District Pediatric Society. When not seeing patients, Dr. Dvorscak enjoys photography, running, and spending time with her family.

Kristina M Lahtinen-Aley, MD

Dr. Lahtinen-Aley received her BS Degree from Colby College in Waterville, Maine in 2000. She attended Medical School from 2000 to 2004 at Albany Medical College earning her MD degree in 2004. Albany Medical College gave Dr. Lahtinen-Aley letters of Commendation for Academic Excellence in Healthcare and Society for two years. Her internship and residency took place from 2004 to 2007 at Albany Medical Center Hospital. During that time she was a coordinator of the "Reach out and Read" program which educated parents on the importance of reading to their children and distributes age appropriate books to children at their well visits. Dr. Lahtinen-Aley is Board Certified by the American Board of Pediatrics. She began working at Community Care Physicians with Latham Pediatrics in 2007. Her interests include not only well-child care from newborns to adolescents, but she also focuses on international and domestic adoption. She enjoys the outdoors, athletics, and traveling.

Jessica L Lawson, NP

Jessica Lawson, RN, MSN, CPNP-PC is a Certified Pediatric Nurse Practitioner. She graduated from Russell Sage College with a BA in Biology (2000) and a BS in Nursing in 2006. Jessica's professional career began in 2007 as a Registered Professional Nurse at Albany Medical Center Hospital in the Neonatal Intensive Care Unit. In 2011 she was promoted to Assistant Nurse Manager and remained in that position until 2013. During her employment at AMCH Jessica worked towards her Masters in Child Health Nursing and received her Masters in Child Health Nursing in 2012 from Stony Brook University. Jessica is certified by the Pediatric Nursing Certification Board. Jessica enjoys taking her dog Cooper to the dog park, walking, singing, and spending time with her family.

Board Certifications

Theodore Eugene Talma, MD

Dr. Talma earned his medical degree, MBBS from the University of the West Indies. He served as the House Officer and Senior House Officer during his internship at Queen Elizabeth Hospital in Barbados, and was a junior and Senior Resident at the Hospital for Sick Children in Toronto, Canada. He continued his Residency at the Children's Hospital of Eastern Ontario and later was a Pediatric Oncology Resident at Princess Margaret Hospital in Toronto. He finished his Residency at the hospital for Sick Children in Toronto. He is a Board Certified Pediatrician, a Diplomat of the American Board of Pediatrics, a member of the West Indies Alumni Association and a past president of the Capital District Pediatric Society. Dr. Talma enjoys music and learned to play the guitar as an adult. A few years ago, he started ballroom dancing and has competed successfully in competitions.

Winston Chun-Pon Wong, MD

Dr. Wong received his medical degree from the University of Hong Kong. Dr. Wong's Internship in Internal Medicine and Obstetrics took place at Queen Mary Hospital in Hong Kong. He continued his residency in Internal Medicine at the New England Deaconess Hospital and Joslin Clinic in Boston and completed his residency in Pediatrics at the Children's Hospital in Boston. He also completed his fellowship in Pediatric Nephrology at Cornell University Medical College. Dr. Wong is Board Certified by the American Board of Pediatrics and by the American Board of Pediatric Nephrology. He is a Fellow of the American Academy of Pediatrics and a Clinical Professor of Pediatrics at Albany Medical College. He was a past president of the Capital District Pediatric Society and the Albany Chapter of the Chinese American Medical Society. His interests include clinical nutrition and physical fitness, downhill skiing, golf, gardening, and traveling.

Our Services

Our providers and staff are proud to work in a field specifically dedicated to the care of people during a very important stage of life: childhood. Pediatrics is the medical specialty entirely focused on the physical, emotional, and social, health of children from birth through adolescence. The focus of pediatrics is preventative healthcare and the proper development of children.

Pediatricians are recertified periodically to keep current on changes in children's healthcare.

Our providers are committed to the healthcare of children in the Latham area. Our doctors are also the school physicians for the Colonie School District. They complete physical exams on site at the schools.

Connect With Our Office Online

Latham Pediatrics has implemented a patient portal to provide patients with a new option for connecting with their provider. This portal, called mycareDOT, offers patients and providers a wide array of services to facilitate health information exchange, including: personal health record (PHR), requests for appointment scheduling and referrals, patient health education and electronic health prescribing.

To enroll in mycareDOT, you must first be an established patient of our practice. Visit www.mycaredot.com to set up a new user account. After setting up a new user account, you then proceed through the set up wizard, where you will be prompted to choose your child's pediatrician with whom you will communicate. From there you can update your child's personal health record, including medical problems, medications, allergies, family and social history, and immunizations.

In mycareDOT, before you can communicate with the doctor's office, the provider must first accept or decline the online relationship with you. Once accepted, you can send general messages to your child's provider, request/cancel an appointment, request a prescription refill, or request a referral. All of these services are free of charge. From the comfort of your own home, office or any location with Internet access, you can now communicate with our office at any time for routine questions.

Telephone Advice

Many illnesses, symptoms, and problems may not always require an office visit. Some issues can simply be resolved or discussed with a phone call to one of our triage nurses. Our triage nurses have been working in the practice for many years and know the ins-and-outs of pediatric patient protocols. Most questions can be answered right over the phone.

If they think that the question requires more assistance or knowledge they will consult with one of our providers and call you back with an answer. Of course if you feel your child should be seen, give our receptionists a call to schedule an appointment: (518) 783-3110

Emergency and After Hours Phone Calls

A provider is always available to take on call phone calls after hours. When you call our answering service will take a message, and the message will then be forwarded to the provider. Our answering service staff are not medical personnel, thus they are not permitted to give medical advice or make medical judgments. If your call is urgent, please let our answering service know, so they can contact the provider immediately. If your call is not an emergency, but cannot wait until regular office hours, the answering service will contact the on-call provider. The on-call provider should get back to you within one hour. Please try to plan ahead with routine medication refills, as medications are not prescribed over the phone, you can also request a medication prescription renewal using our patient portal.

If you feel that your child's problem is life threatening, we ask that you calmly call 911 or go to your nearest emergency room. The physician in the emergency room will notify our office that you have been there.


If an emergency arises and your child needs to visit the emergency room, your child's primary care provider or the provider on call will be able to direct you to an appropriate emergency room based upon their symptoms. Both Albany Medical Center Hospital and St. Peter's Hospital are utilized by the providers at Latham Pediatrics. In the event of a hospitalization, your child would be cared for by the AMCH Hospitalist Group or the St. Peter's Hospitalist Group. If your baby was born at Bellevue Hospital, your baby cannot return there for emergency care. Bellevue is not an emergency facility, and only takes care of the birthing process.

Walk-In Visits

Here at Latham Pediatrics, we frown upon "Walk-In" visits. Please call to make your appointments, or use our patient portal (not available for same day appointments). For us to see all of our friends on any given day, we must have all available appointments open to be scheduled. We are devoted to seeing all of our sick friends as soon as we can, which most days is the same day you call.

School and Camp Physical Forms

We ask that at the time of your child's physical or well visit that, you provide us with any special forms that your child may need for camps, school, sports, etc. The forms can be given right to your nurse and we can fill them out while you are here. If you have forms for us another time, they can be dropped off at the front desk to be filled out by our providers. Forms can take up to one week to complete, we will call you if your forms are done any sooner. When you are picking up your forms, there is a $3.00 fee per form that can be paid by cash, check, or credit card.

Controlled Substance/ADHD Prescriptions

Please allow up to three business days for controlled substance or ADHD medication prescriptions to be completed. Your child's med-checks must be up to date to receive a prescription (every 3 months). These prescriptions will be prescribed electronically to the pharmacy of your choice.

Insurance/Billing Policies

To prevent any confusion between our patients and our practice we have drafted the following financial policy. Please take a moment to review the policies and if you have any questions, please feel free to contact the billing department. A complete understanding of our policies is essential to quality care and service to all of our patients. A comprehensive understanding to your financial responsibilities is essential in what we hope is a long standing and healthy relationship with our office and it's staff.


Our office participates with most major insurances. We ask that you bring your insurance card and a photo identification card every visit.

Health insurance today is vaster and has more options than ever. Each insurance plan is different and it is important to be familiar with your child's individual insurance plan and benefits including: co-payment, co-insurance, deductible, non-covered services, and restrictions.

If our office does not participate with your insurance we still gladly see your child. We request the payment in full at time of service, and you are responsible for fees associated with the visit such as lab fees, x-rays, etc.


All co-payments are due at the time of the visit. Failure to pay the co-payment at the time of visit will result in a $20 surcharge plus the co-payment.

Responsible Party

We ultimately hold both parents responsible for payment of any visit associated fees. In situations where the parents of the child are separated or divorced, Latham Pediatrics will not act as the mediator in collecting payments. If a divorce order states that the other parent has to pay all or part of the treatment costs, it is the parent's responsibility to collect the payment from the other parent. The court order is between the parents of the child and does not pertain to Latham Pediatrics. If the balance on a child's account is not resolved in a timely manner, both parents will be sent to our collection agency.

Please complete these forms prior to your scheduled appointment and mail them back to our office. If your appointment is scheduled in fewer than 5 days, please fax the forms to our secure fax, 518-783-7506, or bring them with you to your scheduled visit.
