The Pediatricians at Clifton Park Pediatrics are dedicated to providing our patients with comprehensive pediatric care, from birth through to college. We are conveniently located on Route 9 in Clifton Park. Our practice's team of professionals is committed to assisting you and your children with any healthcare concern you may have.
Chances are you'll visit your child's pediatrician most often during the first year of his or her life for routine visits. However, regular visits to the doctor are necessary at every age. We encourage you to speak with your doctor at Clifton Park Pediatrics about what visits are needed at what time in order to maintain optimal health for your child.
Our staff is specially trained to get you the answers you need to assist with your family's healthcare needs. Our nurses are committed to educating patients about their medical conditions and working with them to reduce their health risks. Our office focuses on prevention and disease management, in addition to treating acute issues that arise.
During non office hours an on-call pediatrician is always available. We are also affiliated with local Community Care Physicians' Urgent Care Centers if you need to be seen at a time when our office is closed.

As a patient of Clifton Park Pediatrics, we want to make sure you have convenient access to our office and staff. Our patient portal is a convenient way to communicate with us and access part of your medical record from a computer. This online patient portal is called mycareDOT™, powered by FollowMyHealth®, and it complements your traditional visits with our doctors.
The mycareDOT™ patient portal allows you to contact our office 24/7 using a secure internet connection. With just a few clicks you can:
mycareDOT™ is free, easy, and convenient. It's not meant to replace face to face visits, but complements your traditional appointments and you don't have to be super tech-savvy to use it. All you need is a personal email and an internet connection. There are good reasons to give the portal a try.

A little preparation will help us to serve you better. Bring to your child's appointment their health insurance identification card and your pharmacy plan insurance card. Bring also a form of picture identification, like a driver's license (yours, if your child is under the age of 18).
If your child is taking medications, either prescription or non-prescription, bring their pill bottles to our office.
Come to the office a little before the scheduled appointment so that we can update your child's personal information in plenty of time.
Please check-in at the reception desk as soon as you arrive.
We strive to provide your child with the medical care that they need, when they need it, as comfortably and as conveniently as possible. All of our health care providers are trained and certified in the evaluation and treatment of all aspects of pediatric health care. To make an appointment, simply call our office during normal business hours at 518-371-5437. You can now also request appointments using our patient portal, mycareDOT™, powered by FollowMyHealth®.
You select your children's primary care physician. Their primary care physician provides them with regularly scheduled medical care and will also provide care for illnesses or injuries that arise between scheduled visits.
If your child is ill, and their primary care physician is not available, another Pediatric provider in our practice will evaluate and treat him or her. If the problem requires further care, you will be given an appointment to follow up with your child's primary care physician. Their complete medical record is available to each provider within Community Care Physicians, so all are informed with the information they need to care for your child.
If you must cancel an appointment, we ask that you do so at least one day before the appointment. We are happy to reschedule your appointment.

We call every patient's contact phone number before their appointment to verify that they will attend.
The medical provider makes every effort to see you and your child on time. This is usually within 15 minutes of your appointment time.
Occasionally your provider may be delayed. If so, it is because the health condition of other patients has required more time than your provider anticipated.
Our providers will afford each patient the time that is needed to attend to the health problems at hand. All patients will receive the same attention.
Please be patient if the provider is delayed. If the provider is occupied with a serious health emergency, you will be given the options to wait for the provider, to see an alternate provider, or to reschedule the visit.
If you have not been called from the waiting area within 15 minutes after your scheduled appointment, please return to the reception desk. If at any time you have questions as to if the provider is running on time, please ask us!
Prescription medications can be an important part of your plan of care. To dispense your child's medications safely and effectively, follow a few simple suggestions.
Please bring all your child's pill bottles with you to EACH appointment so that we can verify the names and doses of their drugs, their expiration dates and number of refills available. Keep all medications in accurately labeled bottles or containers.
We want to refill at your office visit any prescriptions that will expire before your child's next scheduled visit. (Bringing their pill bottles helps us to determine which medications need refills). You can now also request prescription refills using our patient portal, mycareDOT™, powered by FollowMyHealth®.
Your primary care provider may recommend that your child be evaluated and treated by a specialty physician if their medical condition warrants. Community Care Physicians includes a number of excellent specialty and subspecialty physicians, all of whom share electronic health records and imaging studies allowing your child's records to be immediately accessed by the providers administering their care. We are happy to refer you to any of these Community Care specialists when needed or to any appropriate specialist you prefer.
Our office will provide notifications to, or request authorization from your health insurer if these are required.
We will provide the consultant with your child's relevant medical history so that he or she is aware of your child's medical problems.
Payment is expected at the time of service.
If your insurance requires that you pay a co-payment, they require us to collect it. For your convenience we accept cash, checks and most major credit cards. If we must bill you for the co-payment, an additional processing fee is charged.
Please complete these forms prior to your scheduled appointment and mail them back to our office. If your appointment is scheduled in fewer than 5 days, please fax the forms to our secure fax, (518) 383-6737, or bring them with you to your scheduled visit.
