As a new patient, the Patient Connector will work closely with you to find a primary care doctor specializing in internal medicine (age 17 and older), pediatrics (birth to age 18), or family medicine (patients of all ages, birth to geriatrics). Community Care Physicians is the largest independent multispecialty medical group in the Capital Region and has many offices across the area. Instead of calling multiple offices and compiling information on individual providers, you can simply call The Patient Connector through the Phone Concierge service to find a doctor that is right for you and/or your family.
Ease of Service
The Patient Connector will walk you through a series of questions, including, but not limited to, where you live, how far you are willing to travel, if the doctor is for you or your family (including children) and if you prefer a male or female provider.
