Community Care Physicians is focused on the physician-led model of medicine. We are a physician-owned and managed Professional Corporation governed by a Board of Directors comprised of physicians. As such, we are guided by those who are practicing in our offices every day and the daily practice of medicine is focused on creating an optimal experience for the patient and physician. CCP embraces the independent practice of medicine and provides framework and support that allows doctors to focus on what they do best – practice medicine. Our organization has been highly successful over the past 30+ years. We encourage practitioners to participate in decisions, so we can remain focused on how our goals and objectives impact our team and our patients.

Our business model is attractive to those who find the cost of billing, information systems, regulatory compliance, practice coordination and other expenses to be overwhelming for a small practice. It is also appealing to larger practices that prefer to maintain their independence rather than join a hospital-based system.

Our Commitment to Quality


  • 25 practices currently participate in a Patient- Centered Medical Home initiative, in partnership with one of our regional payers.
  • Our primary care practices have been recognized by the National Committee for Quality Assurance (NCQA).
  • Our physicians work side-by-side with nurse practitioners, physician assistants, nurses and medical assistants to deliver team-based care, which enhances care for the patient.
  • We have participated in pilot programs to reduce avoidable hospital readmissions, enhance generic prescribing and provide access to a clinical pharmacist and care mangers within the office.

Our Commitment to Innovation


  • CCP’s medical practices belong to a single integrated health system, sharing a single practice management and electronic health records platform, Allscripts.
  • CCP utilizes a patient portal called FollowMyHealth™ to connect with patients securely online.
  • CCP is a founding member of HIXNY, the Regional Health Information Organization, and shares clinical data electronically with hospitals, health plans and other physicians throughout the region.
  • We have developed and integrated technologies into our work-flows, including VICA - Virtual Care, a secure smart phone application that allows for virtual video visits with our patients, and also TeleCare, a hands-free virtual scribing system that allows practitioners to work more efficiently and spend less time after hours with patient charts.

Our Commitment to Community


  • We support many regional organizations through sponsorships and employee participation, including the American Cancer Society, MS Society, Alliance for Positive Health, American Heart Associations, Albany Symphony Orchestra, and Albany Pro Musica, among others.
  • Our physicians have participated in medical missions to Uganda, Kenya, Haiti, and the Dominican Republic.
  • We serve as physicians for local schools and area sports teams.
  • We are committed to educating our communities about health through free educational seminars and events.
